Much spinal imaging is acquired lying down. That is appropriate for some questions and misleading for others, because it removes the load that produces your symptoms in the first place.

Standing and whole-body views show alignment under load — how the spine, pelvis and lower limbs actually stack when gravity is acting on them. Leg length difference, pelvic obliquity and compensatory spinal curvature are frequently invisible on a supine study and obvious on a standing one.

This matters for patients whose pain is mechanical and load-related, where the question is not what a single segment looks like in isolation but how load is distributed through the whole chain.

X-ray shows bone well: alignment, degenerative change, fracture, spondylolisthesis, hardware position and joint space. It is quick and low-dose.

It does not show soft tissue well. Discs, nerve roots, ligaments and muscle are largely invisible, which is why a normal X-ray after an injury does not mean nothing is wrong — a point that matters particularly after a motor vehicle accident.

Where nerve involvement is suspected, electrodiagnostic testing answers a question no image can: whether the nerve is actually functioning abnormally, rather than whether something nearby looks abnormal.
