State medical licensure is not administrative trivia, and its practical effects show up in ways patients rarely anticipate until they hit one.
A physician licensed only in Missouri generally cannot prescribe for you while you are at home in Illinois, cannot conduct a telehealth follow-up with you across the state line, and has no standing if you are admitted to an Illinois hospital. Each of those becomes a handoff to someone who has not met you.
Being licensed in both states removes all three frictions. Prescriptions, follow-up and continuity work the same whether you are in Madison County or St. Louis County.

We would rather state that plainly than imply a presence we do not have. If a same-city office is your priority, that is a legitimate thing to want and we are not it.
This is the question that causes the most avoidable trouble, so it is worth setting out properly.
Most major commercial plans and Medicare travel across state lines without difficulty. Medicare in particular is a federal program, and a Missouri provider is a Missouri provider whether you live in Edwardsville or Ellisville.

Which bridge you take matters more than the mileage. From Madison County, I-270 west across the Chain of Rocks crossing feeds almost directly toward Lambert Airport and our Woodson Road office, and it avoids downtown entirely. From St. Clair County, I-64 west over the Poplar Street Bridge then north on I-170 is usually faster outside peak hours but puts you through the downtown interchange.
Neither route is long. The thing worth planning around is the afternoon peak on I-270 westbound, which can add twenty minutes for no good reason. Morning appointments generally travel better from the Illinois side, and our schedulers know this and will offer them if you say where you are coming from.
No. Many patients refer themselves. Some plans require a referral for coverage, which is a separate question we will check.
Yes. That is one of the practical consequences of holding an Illinois license.
