The Woodson Road office is about eighteen miles from central Chesterfield. Page Avenue east to I-170 north is usually quickest; I-64 to I-270 north is the alternative when Page is congested. It is a real drive and worth being honest about.

Why injections fail, and what to do about it

The most common reason patients travel to us from west county is that injections were tried and did not help. That experience is usually interpreted as “injections do not work for me.” It is more often a targeting problem, and the distinction matters because the two lead to completely different next steps.

There are four recurring explanations, and they are separable:

1. The wrong structure was treated. Low back pain can originate in a disc, a facet joint, a sacroiliac joint or a nerve root, and these produce overlapping symptoms. An injection aimed at the facet joints will not help pain coming from the sacroiliac joint, no matter how well it is performed.
2. The diagnosis was never actually confirmed. If no diagnostic block established the source before the therapeutic injection, the target was an educated guess. Sometimes the guess is right. When it is not, the injection fails for reasons that had nothing to do with the medication.
3. The injection did not reach the target. Without image guidance a meaningful proportion of injections land somewhere other than intended. Under fluoroscopy the needle position is confirmed and contrast shows where the medication actually spreads.
4. The pain is being driven centrally. After long enough, the nervous system itself can become sensitized and amplify signals out of proportion to any remaining tissue problem. Injecting the original structure will not fix a problem that has moved upstream — that needs a different approach entirely, sometimes including ketamine for established central sensitization.

Making one trip do the work of two

Because of the distance we try to compress the sequence. Where a diagnostic procedure is clearly indicated and you want to proceed, it can often be performed the same day as the consultation rather than requiring a second visit.

To make that possible, bring any imaging you have — the discs or a patient portal login, not just the report — a list of what you currently take, and a short account of what has already been tried and what each attempt did. If a previous injection helped briefly, that is diagnostically useful information and worth recalling before you come.
