This policy explains what Padda Institute collects through this
website, how we use it, and the choices you have.

Padda Institute is operated by INTERVENTIONAL CENTER FOR PAIN MANAGEMENT, P.C. References to “we,” “us,” and “our” below mean that entity.

What this policy covers

This policy covers information collected through painmd.tv and the ways we communicate with you about it. It is separate from the HIPAA Notice of Privacy Practices below, which governs your protected health information as a patient.

What we do not do

We do not sell your personal information. We share information with service
providers who work on our behalf — and only as far as they need it to do that work.

No mobile information will be shared with third parties or affiliates for marketing or
promotional purposes.
Information sharing with subcontractors who provide support services
on our behalf — such as our SMS delivery provider — is permitted only so far as they need
it to deliver messages for us. Text messaging originator opt-in data and consent are excluded
from all such sharing, and that information will not be shared with any third parties.

Cookies

You can refuse or delete cookies through your browser settings. Parts of the site may not work
properly if you do.

Security

We use administrative, technical and physical safeguards to protect the information we hold. No
method of transmission over the internet is completely secure, and we cannot guarantee absolute
security. Please do not send health information through this website’s forms, by email, or by
text.

Children

This website is not directed at children under 13, and we do not knowingly collect their
information online.

Your choices

You may ask us what information we hold about you, ask us to correct or delete it, and opt out of
messages at any time. Contact us using the details at the end of this page.


HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW
YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Padda Institute is required by law to maintain the privacy of your protected health information (PHI), to give
you this notice of our legal duties and privacy practices, and to follow the terms of the notice
currently in effect.

How we may use and disclose your health information

Other uses and disclosures require your written authorization, which you may revoke at any time in
writing. This includes most uses for marketing, any sale of your

, and most uses of psychotherapy notes. If we contact you for
fundraising, you may tell us to stop.
