What Actually Happens During a Piriformis Injection

Most of what makes a piriformis injection go well happens before the needle moves. This is what the procedure
involves at the Padda Institute, in the order you will experience it, including the parts that are less pleasant
and the risks that are worth knowing about in advance.

Before the day

Two things get settled first. The first is whether the piriformis is genuinely the target — an injection
into the correct muscle for the wrong diagnosis is a wasted procedure and a misleading result. The second is
whether anything makes the injection unsafe for you specifically.

The contraindications are straightforward. Active infection, either at the injection site or systemically, and
clinically significant coagulopathy or anticoagulation that cannot be safely managed around the procedure, both
rule it out. So does a known hypersensitivity to the planned injectate, the contrast agent, or the other
procedural medications.1 If you take a blood thinner, that conversation needs to happen at the visit
before the procedure, not on the morning of it.

No sedation, and why that is deliberate

We do not use sedation for this procedure. You are awake, you are positioned face down, and you are able to
respond throughout.

That is not a cost decision. A piriformis injection is performed millimeters from the sciatic nerve, and a
patient who can say “that just went down my leg” is providing information no monitor supplies. Sedation removes
exactly the feedback that makes the procedure safer. The practical consequence for you is that you drive yourself
home afterward and you do not need to arrange a ride.

The needle is advanced in stages, with a check image between movements rather than a single continuous push. This is what image guidance actually looks like in practice — a series of small corrections, not one confident pass.

The sequence is short and predictable. The skin over the buttock is cleaned and draped. Local anesthetic is
placed in the skin and along the needle track, which is the only genuinely sharp part and it lasts seconds. The
procedure needle is then advanced toward the muscle under image guidance, checked, adjusted, and checked
again. We use both fluoroscopy and ultrasound, and prefer fluoroscopy where it is available — because a
small volume of contrast injected before the medication shows where that medication will spread, which
ultrasound cannot document in the same way.

Once contrast confirms the placement, the injectate is delivered. Most patients feel a deep pressure or a
cramping ache at this point as the muscle accepts volume; it is the sensation people remember and it settles
within a minute or two. The needle comes out, a small dressing goes on, and you are helped up.

Published procedure times give a realistic sense of scale: in a cohort of 109 piriformis injections, median
times were eight minutes with fluoroscopy, ten with ultrasound, and eleven with both combined.2 The
appointment is longer than the procedure, but not by as much as most people expect.

After

  • Recovery is two to four hours, after which normal activity, including work, resumes.
  • You drive yourself home, unless the leg is temporarily numb or weak, in which case we keep
    you until it clears.
  • Soreness at the site for a day or two is expected. Ice helps. It is not a complication.
  • Keep the site clean and dry for the rest of the day.
  • Track your response. Write down what changed and when — the first few hours behave
    differently from the following week, and that difference is diagnostically meaningful.

The risks, named

Complications of piriformis injection are uncommon, but the honest list includes bleeding, infection,
post-injection soreness, transient worsening of pain, headache, allergic or other adverse reactions to the
injectate, and failure of the procedure to provide meaningful relief.1 That last one belongs on the list.
It is a possible outcome, not a malfunction.

The location-specific risk is the sciatic nerve. Because of its proximity and that of the surrounding gluteal
neurovascular structures, inadvertent nerve irritation or injury remains an important procedural risk,
particularly when needle localization is inaccurate — and fluoroscopic or ultrasound guidance improves
needle placement and may reduce that risk.1 In the 109-procedure cohort, the most common adverse effect
observed was transient leg weakness or numbness.2

How many injections

The number is decided by clinical need, not by a schedule. There is no particular magic in a series of three,
and we do not pre-book a course of injections before knowing how you responded to the first one. If a repeat is
indicated it is typically some weeks later, it is limited by cumulative steroid exposure, and the plan is
reassessed each time rather than assumed.

The skin and the needle track are numbed with local anesthetic first, so the sharp part is short. The sensation most patients report as the memorable one is a deep, cramping pressure as fluid enters the muscle belly — uncomfortable for under a minute, not sharp. No. We do not use sedation for this procedure, so you are awake and clear-headed throughout and you drive yourself home. How long does the procedure take?

The injection itself is short. In a published cohort of 109 piriformis injections, median procedure times were eight minutes under fluoroscopy alone, ten minutes under ultrasound alone, and eleven minutes for the two combined, with the longest cases in any group running to roughly 25 to 28 minutes.
